Something is seriously broken about the way combat concludes (bugs and speculation inside)

So I have come across multiple bugs throughout my play-through so far, and the one thing they all have in common is that they occur (or in some cases the thing that should occur doesn't) right when combat is finished.

 

First, the bugs:

 

  1. At some point, Bonded Grief stop disappearing after combat and became a constant effect on Sagani. The only way to remove it is to remove and add her back into the party. Normally this effect should be cleared as soon as combat ends.
  2. During the quest "Supply and Demand" the journal does not update after defeating Aefre, making the quest impossible to complete when choosing to fight her. Presumably the journal should be updating as soon as the fight with her is concluded.
  3. During the quest "At all Costs" the journal does not update after defeating Mestre Verzano, making the quest impossible to complete when choosing to fight him. Presumably the journal should be updated as soon as the fight with him is concluded.
  4. During the quest "Lord of a Barren Land" the journal does not update after defeating Lord Raedric, making the quest impossible to complete when choosing to fight him. Presumably the journal should be updated as soon as the fight with him is concluded.
  5. When resolving stronghold invasions manually, instead of the screen fading to black as it should, my party simply remains on the battle field. At first glance this did not even seem like a bug, however, on every transition back to that map (in my case the great hall), the invaders will have re-spawned. The same thing happened upon the next invasion (which i had to do while also fighting enemies of the previous invasion, as they were still present). When entering the great hall now i am faced with both groups of invaders at the same time, and they re-spawn every time my party enters.

While these bugs could all be isolated problems, the one thing they all have in common is that some scripted event is supposed to occur when the combat state of the party is resolved. This led me to the conclusion that all of these problems could be connected, i could however be entirely wrong.
