If you go to eternity.obsidian.net, there should be a place under My Products to redeem your key. You’ll need to select either GOG or Steam and then use that key to unlock the season pass for your game provider of choice

You can start a new game or reload a pre-Ukaizo save.

You need to have a Game Center account first. The merge process is hanging because it’s looking for an iOS account to integrate and coming back with nothing. Once you have two accounts (iOS and Steam) then it will go pretty quickly

EDIT: If they are showing up before you complete the quest, then yes, it's a bug.

This appears to have been resolved.

1) You had to back at or above a specific level
2) You had to fill out the survey they sent, specifying how you wanted your name to appear
If both of those things didn't happen, then no name in the credits.
